The Center is a fully-licensed 82-bed Skilled Nursing and Rehabilitation facility certified to provide care under both Medicare and Medicaid programs. The Center is one of the few not-for-profit, long-term care facilities still serving the residents of Rensselaer and Southeastern and central Washington Counties in New York, Southwestern Vermont and Northwestern Massachusetts. We work very hard at every level to achieve excellence for both our residents and staff.
Time and time again, it is the combination of the services and the care we provide that has families returning to The Center and telling others about us. Although we are a small facility, we are a facility that works very hard at making each resident’s stay here individualized, safe and fun.

Hi Everyone,
Our hope with the strategic plan is to create clear and measurable goals for the next 3-5 years and very specific initiatives to accomplish those goals. Focused on an ever-increasing need to become more competitive within our industry, this plan paves the way for us to achieve two initiatives: to become the “supplier of choice” for individualized health care and the “employer of choice” in our area.
